- Request that we correct your PHI if it is incomplete or incorrect
- Ask us to restrict how we use and share your PHI
- To request that we send your PHI to another person or organization
- Revoke your authorization to use or share your PHI

CHANGES TO THIS NOTICE
We reserve the right to change the terms of this notice and to apply the changes to PHI we already have about you. We will notify you of any material changes to this notice.
